An opportunity to share your thoughts!
The transition from high school to the next phase of life can be a challenging time for youth. Resources, guidance, tips and tools are ever more important in trying to find the best way to navigate through this time of change for youth with a disability and their families.
In light of this, Plan Institute is reviewing its Beyond Graduation workshop in hopes of building a better set of tools to help guide families during this time of transition. We are asking families to participate in a short survey to share their experiences in order to help guide other families who may be going through, or who will soon be going through, a similar process.

Families who are currently going through this transition period, as well as those who have already made the transition, are encouraged to contribute. Please feel free to share this survey with others in your community that may be interested in participating. Survey closes May 30th, 2014.
